Starting with Blackwhip in chapter 210, Deku then began to manifest One for All's stored quirks. It turns out that the original owners of these two quirks didn't fully trust Deku at the time. My Hero Academia exposed every One for All Quirks through the time of Dark Deku's vigilante era where he had won them over, with the Third User's Fa Jin as the last one to be revealed to readers. The absence of the Second User's Quirk was not fully explained until chapter 349, when Deku tried to use it but was instructed to hold it back as a 'last resort' without any further explanation. The revelation of each quirk for One for All made users realize how dangerous Deku's final quirk would be.
In chapter 368, as Deku engaged in the hybrid Shigaraki/All for One entity, the 'last option' scenario finally materialized. 'The Second's Transmission' is the name of his attack that he naturally yells, removing any confusion, then starts to punch Shigaraki/All for One, ratcheting up the ability from Second Gear to Third Gear to Top Gear to its final power, unleashing a Detroit Smash that, according to the Second User's vestige, is equivalent to about 120% of One For All's power. When everyone finally understands what is happening, All for One's younger brother, Yoichi Shigaraki, who was the first user of One for All, makes an appearance and tells them their intention is to finally put a conclusion to this long-lasting family struggle.
The second user's quirk, Transmission, changes the speed at which things it touches move. It seems to act as a power multiplier by creating a large and sudden acceleration. Deku's 100% level was exceeded, which was really an incredibly powerful attack. Thus, the Second User warned that as more power was accumulated within One for All, his power had mutated and increased, making it extremely harmful for the user before using it in 349. Now that it's been used, it makes clear that Deku was regularly breaking his bones long before he even came close to using 100% of One for All's strength. As such, 120% is undoubtedly going to put his life in danger.
With a five-minute warning, the Second User's Quirk, Transmission, would force a hard cap on Midoriya's fighting time simply due to the damage given. Not only that, it could also be even more damaging to Deku and his opponent if it was combined with other strikes from My Hero Academia that increase output, for example, Fa Jin.
